1
Meaningful FSHR Variation Across the UPMC Network: 13.3% to 66.7%
The system-wide weighted average FSHR is 31.4%, but individual hospital performance ranges from 13.3% (UPMC Western Maryland) to 66.7% (UPMC Kane). This 53.4 percentage point spread represents significant, actionable variation in post-acute network performance that is not yet fully visible within UPMC's current care management infrastructure.
2
15 SNF Partners Account for Concentrated Hospitalization Risk
15 priority target SNFs serve 748 patients system-wide with above-average FSHR rates. The top target, Spring Creek Rehabilitation And Nursing Center, carries a 48.5% FSHR — 17.1 percentage points above the system average. These facilities represent the highest-leverage engagement opportunities for UPMC's post-acute network management program.
3
25 Cross-System SNFs Create System-Level Engagement Opportunities
25 SNFs receive patients from 2 or more UPMC hospitals, creating system-level leverage for preferred network conversations. The highest-volume cross-system SNF, Vincentian Home, serves 142 patients across 3 hospitals. Coordinated engagement at these facilities allows UPMC to move network-wide performance with a single partnership conversation.
4
Limited Visibility Across 25 Hospitals: 174 Destinations, Partial FSHR Ratings
UPMC's 25 active hospitals collectively discharge to 174 distinct SNF destinations. Many SNF relationships lack facility-level FSHR data due to CMS suppression thresholds for small-volume relationships. This limited visibility constrains UPMC's ability to systematically identify underperforming partners and build a high-performing preferred post-acute network. Structured SNF engagement and preferred network designation would create the monitoring infrastructure needed to close this gap.
5
3 Hospitals Below CMS Reporting Thresholds — Data Gap to Address
UPMC Passavant – Cranberry, UPMC Greene, and UPMC Muncy fall below CMS Medicare FFS claims volume thresholds for this reporting period. These facilities have no FSHR-rated SNF relationships in the data. As UPMC's post-acute network management program matures, ensuring these sites are included in preferred network initiatives and tracked through internal data will be critical to comprehensive system performance management.

1
Launch Priority Engagement with Spring Creek Rehabilitation And Nursing Center and Jamc Opco Llc
These two facilities carry the highest FSHR rates in the UPMC network and serve patients from multiple UPMC hospitals. An immediate clinical liaison visit, aligned with a preferred network conversation, is the fastest path to FSHR reduction at scale. Target: achieve preferred network designation within 90 days of initial contact, with quarterly FSHR review milestones.
2
Build a UPMC Preferred Post-Acute Network Around the 25 Cross-System SNFs
The 25 SNFs that serve patients from 2+ UPMC hospitals are the foundation of a scalable preferred network. A single engagement at Vincentian Home (142 patients, 3 hospitals) moves multiple hospital FSHRs simultaneously. Establish shared performance expectations, quarterly scorecards, and a preferred status designation for facilities meeting UPMC's quality criteria.
3
Prioritize UPMC Jameson, UPMC Kane, and UPMC Altoona for Immediate Network Rationalization
These three hospitals show the highest system FSHRs (66.7%, 42.6%, and 54.3% respectively). A structured preferred network review — identifying which SNF destinations carry elevated FSHR and funneling volume toward lower-risk partners — will have the highest near-term impact on system-wide performance.
4
Implement FSHR-Based SNF Scorecards Across All 25 Active Hospitals
UPMC currently has 174 SNF destination relationships with uneven FSHR visibility. Quarterly SNF scorecards — anchored to FSHR, volume share, and facility risk — would give care coordinators and discharge planners a consistent performance signal for every post-acute partner. Puzzle Healthcare's preferred network platform can automate scorecard generation and alert teams to FSHR changes in real time.
5
Address Data Gaps at UPMC Passavant – Cranberry, UPMC Greene, and UPMC Muncy
Three UPMC hospitals fall below CMS Medicare FFS reporting thresholds for this period. Supplement federal claims data with internal utilization data to develop SNF performance profiles for these sites. As UPMC's Medicare FFS volume at these campuses grows, they should be integrated into the preferred network program from the outset to avoid the reactive engagement cycles seen at higher-volume hospitals.
SNFs Serving 2+ UPMC Hospitals
These 25 facilities represent system-level preferred network opportunities. A single engagement at any of these SNFs moves FSHR for multiple UPMC hospitals simultaneously. Sorted by total patient volume descending.

FSHR = Facility Hospitalization Rate from CMS Medicare FFS Claims. Weighted FSHR calculated across all UPMC hospital relationships for each SNF. System FSHR benchmark: 31.4%. Color coding: red = FSHR ≥36.5% (+5pp above system); amber = ≥33.5% (+2pp); green = ≤28.4% (−3pp).

Priority SNF Engagement Targets
Ranked by priority score combining FSHR above system average, total patient volume, and multi-hospital presence. FSHR threshold for inclusion: significantly above system average (31.4%). These 15 facilities represent the highest-leverage opportunities for UPMC's post-acute network engagement program.

Priority score = composite of FSHR excess above system (31.4%), patient volume, and multi-hospital appearance count. Est. Preventable Admits = patients × FSHR. Data: CMS Medicare FFS Claims, Q4 2024 – Q3 2025. Download full Excel workbook →

Data Source
All post-acute performance data in this report is derived from CMS Medicare Fee-for-Service (FFS) Claims. This is the same claims data used in CMS star ratings, value-based purchasing programs, and public quality reporting. Data reflects discharges from UPMC Health hospitals to skilled nursing facilities during the period Q4 2024 – Q3 2025.
Facility Hospitalization Rate (FSHR)
FSHR (Facility Hospitalization Rate) measures the percentage of SNF patient-days that result in an acute hospital readmission, calculated at the SNF-hospital relationship level. Unlike simple 30-day readmission rates, FSHR captures rehospitalization across the full SNF stay, providing a more comprehensive view of post-acute performance. The UPMC system-wide weighted average FSHR is 31.4% for Q4 2024 – Q3 2025.
Priority Score Methodology
Priority scores combine three factors: (1) FSHR magnitude above the system average, (2) total patient volume at the facility, and (3) multi-hospital presence within the UPMC system. SNFs appearing at 2+ hospitals receive a multiplier reflecting their system-wide engagement leverage. Scores are relative within the UPMC Health system.
CMS Suppression & Data Availability
CMS suppresses FSHR values for SNF-hospital relationships with fewer than 11 patients in the measurement period. Of UPMC's 174 unique SNF destinations, only those with sufficient volume have FSHR data. Additionally, 3 UPMC hospitals (Passavant – Cranberry, Greene, Muncy) fall below CMS thresholds for hospital-level reporting and are excluded from quantitative analysis. "Suppressed" SNF destinations are counted but not rated in this analysis.
FSHR Color Coding
Throughout this portal, FSHR values are color-coded relative to the UPMC system average (31.4%): Red = ≥36.5% (5+ pp above system); Amber = ≥33.5% (2+ pp above); Green = ≤28.4% (3+ pp below). Uncolored values are within ±2–3 pp of system average.
