Methodology
Data Source
All post-acute performance data in this report is derived from CMS Medicare Fee-for-Service (FFS) Claims. This is the same claims data used in CMS star ratings, value-based purchasing programs, and public quality reporting. Data reflects discharges from UPMC Health hospitals to skilled nursing facilities during the period Q4 2024 – Q3 2025.
Facility Hospitalization Rate (FSHR)
FSHR (Facility Hospitalization Rate) measures the percentage of SNF patient-days that result in an acute hospital readmission, calculated at the SNF-hospital relationship level. Unlike simple 30-day readmission rates, FSHR captures rehospitalization across the full SNF stay, providing a more comprehensive view of post-acute performance. The UPMC system-wide weighted average FSHR is 31.4% for Q4 2024 – Q3 2025.
Priority Score Methodology
Priority scores combine three factors: (1) FSHR magnitude above the system average, (2) total patient volume at the facility, and (3) multi-hospital presence within the UPMC system. SNFs appearing at 2+ hospitals receive a multiplier reflecting their system-wide engagement leverage. Scores are relative within the UPMC Health system.
CMS Suppression & Data Availability
CMS suppresses FSHR values for SNF-hospital relationships with fewer than 11 patients in the measurement period. Of UPMC's 174 unique SNF destinations, only those with sufficient volume have FSHR data. Additionally, 3 UPMC hospitals (Passavant – Cranberry, Greene, Muncy) fall below CMS thresholds for hospital-level reporting and are excluded from quantitative analysis. "Suppressed" SNF destinations are counted but not rated in this analysis.
